ENROLLMENT CHECKLIST
All enrollment forms must be completed for any child to stay at Rocking Horse Daycare. It is the parent's responsibility to keep their child's records up to date with:
- Correct work and emergency numbers
- Names of people allowed to pick up your child
- Physician's name and number
- Immunization records
